It's tossing beads from the balconies to passers-by who bare their breasts.
When the crowd walks by a balcony with revelers they start cheering until the beads, candy, and confetti start flying. This is the before picture.
This is the after picture. Did they get a full collection of blue, purple, lavender, and silver beads? I don't know, but I saw so many people sporting a neckfull.
Here are some more balconies. This one is modern architecture...
...this one a Victorian (or an Edwardian, I can never tell) house. Notice the curved windows.
This one is from the painted lady at Sanchez and 16th. They dumped piles of confetti on the street, dusting the crowd and covering the parked cars.
Next we stop at a red light, just between the oldest church in San Francisco de Asis and a new synagogue
